Update your Contact Details

If you change address, name, home or mobile telephone number please let one of the receptionists know, so that we can update your records. If you move outside your doctor’s practice area you will need to change to a nearer GP.

Please provide proof of your new address if changing your address, this should be a letter dated within the last 3 months. You can email this to gmicb-sal.sides@nhs.net or bring it in to the practice.
