At The Sides Medical Centre, we are committed to supporting the holistic health and wellbeing of our patients. We collaborate with the Health Improvement Team in Salford and the Wellbeing Matters programme to provide social prescribing services that address non-medical factors affecting health.

Social prescribing is a way of connecting people to non-medical support and resources within their community to improve their health and wellbeing. Practice in Salford have a dedicated Community Connector who can support you with non-medical, lifestyle and social needs. This could be finding out what support is out there in your community and choose from a menu of options tailored to you. This could be connecting to social and creative activities, engage in healthy lifestyle behaviours such as physical activity and moving more, connecting to communities of identity and experience, learning a new skill or prepare for employment at your pace, volunteering and much more to improve your health and wellbeing.

Health Improvement Team – Salford
The Health Improvement Team offers a variety of services designed to promote healthier lifestyles and wellbeing. Their programmes focus on areas such as physical activity, healthy eating, smoking cessation, and mental health support. By working closely with individuals, they help set achievable goals and provide ongoing support.

Wellbeing Matters Programme
Led by Salford CVS, the Wellbeing Matters programme connects individuals to community resources through a network of Community Connectors. These connectors work within the five health and care neighbourhoods in Salford, including Swinton, to provide personalised support. They assist individuals in accessing local activities and services that promote independence and reduce social isolation.

How to Access Social Prescribing Services
If you feel that social prescribing could benefit you, please speak to a member of our practice team. We can refer you to the appropriate services offered by the Health Improvement Team or connect you with a Community Connector from the Wellbeing Matters programme. In some instances you can self-refer to these services depending on your needs.
